Hi. This is a sign up page I created in three minutes. You have the option to require people to leave a phone number and/or address. We can tag people that sign up. You can also automatically make yourself the point person for that contact. I'll add a picture below this text to demonstrate what is possible. Using the wysiwyg interface sucks, but it's a lot easier than hand coding something.
And one more thing. You also can customize the autoresponse email someone receives after they sign up.
